Dice your chicken and add it with all ingredients (except cream cheese) to the slow cooker.
Stir it well and cook on low for 6 to 8 hours.
Remove some of the broth and pour it over the cream cheese to soften it. Blend the cream cheese with the broth until all of the lumps are gone.
Pour the mixture back into the soup and stir it well.
